Thumb lifts and O-sign (thumb base exercises)

With your hand resting on its little-finger side, you lift the thumb up, then up and away from the palm, keeping its joints slightly bent, then touch the thumb to each fingertip in turn to make an O. It trains the muscles that steady the base of the thumb, and its assisted version is a gentle start for de Quervain's.

How to move

  1. Warm up: put your hand in a bowl of warm water and gently circle the thumb one way for a minute, then the other way.
  2. Rest your forearm and hand on a table on the little-finger side, thumb on top.
  3. Thumb lift: without help from the other hand, lift the thumb straight up as far as you can, keeping its joints slightly bent. Hold, then lower slowly.
  4. Wide thumb lift: keeping the joints slightly bent, take the thumb as far from the palm as you can while lifting it towards the ceiling. Hold, then lower.
  5. O-sign: rest your elbow or the back of your hand on the table and touch the thumb tip to each fingertip in turn, making an O each time.
  6. Optional web-space stretch: use the other hand to ease the thumb away from the index finger, and hold.

Form cues

  • Keep the thumb joints slightly bent; don't let them bend backwards.
  • Move slowly and keep the wrist still.
  • Avoid: bending the wrist forwards, rushing, or pushing through sharp pain.

Practice and cautions

Hold each lift for 10 seconds, up to 10 times, at least 3 times a week for about 20 minutes a session including the warm-up: level 1 of the OTTER II trial's exercise booklet (the trial's results were not found when this was written). Hold the web-space stretch for 30 seconds, as in an NHS hand-therapy leaflet. For de Quervain's, 10 of each, never into pain, as in another NHS hand-therapy programme. Thumb exercise in thumb base arthritis trials gave mixed results: better than no treatment in the short term, with gains mostly in function rather than pain.

Some slight thumb discomfort afterwards is not unusual but should stop within 24 hours. With de Quervain's, stay pain-free and start with the assisted version: the other hand lifts the thumb away from the index finger and lowers it, then the thumb returns on its own. A hot, swollen thumb base or a new injury needs checking first. This is movement education, not an assessment or treatment prescription. Stop for sharp or increasing pain, dizziness, numbness or tingling; seek individual guidance for symptoms, injury or restrictions.
